Kennel Policies and Contract 

This information will be emailed to your for signature prior to boarding or daycare visit 

• I understand and agree that my dog will be grouped-housed with other animals in their own individual kennel.  

• I understand and agree that my dog must be current on required vaccines - Bordetella, Rabies and Distemper - and that I must be able to provide proof when requested.  

• I understand and agree that my dog has not been ill or injured in the 10 days prior to the start of their scheduled stay. 

• I understand and agree that my dog must be free of fleas and ticks and be on a preventive treatment plan prior to entering the premises. 

• I understand that I may be immediately required to collect my dog at any time during it's stay if my dog poses a threat to itself, other dogs or staff members. 

• I understand and agree that any medication my dog requires must come in a labeled container with the name of the dog, name of the medication and administration instructions. 

• I understand and agree that I must provide my dog's food for the entire stay. If my dog does not have enough food, it will be provided for an additional fee of $5.00 per day. 

• I understand that all personal items provided for my dog are at my own risk and any damage incurred by my dog to their own belongings will be at my expense. 

• I understand and agree that my dog may be given treats during their stay. Please notify the kennel if they should be excluded from this practice. 

• I understand that dogs from the same household may be kenneled together only if they are able to do so without risk of incident or injury. I understand that Yellow Dog Kennel reserves the right to separate these dogs at their discretion due to concerns for the dog's safety. 

• I understand and agree that my dog’s image may be used on social media, video or print, in advertising and as determined by Yellow Dog Kennel and Daycare.

• I understand and agree that my dog must be leashed when arriving at and leaving the facility and that I am responsible for them while outside of the kennel building during drop off and pick up. This includes but is not limited to, any damage done to vehicles, other animals or owners, or the boarding facility. This also includes elopement of my animal from the premises. 

• I understand that I am responsible for cleaning up after my dog should they defecate while outside of the building during drop off or pick up. 

• I understand that pick up and drop off times will be provided to me when scheduling a reservation and that I should arrive during these hours unless a previously scheduled appointment has been made.  

• I understand that, should my dog become seriously injured or ill, Yellow Dog Kennel staff will attempt to contact me and/or my emergency contacts for approval prior to seeking veterinary care. I understand that it may be necessary to seek immediate care due to the severity of the injury or illness at the Veterinarian or Clinic of their choice, with the expectation that I or my emergency contact will be notified immediately. I understand that I am responsible for any costs incurred for treatment. I understand and agree that my dog may be taken off the Yellow Dog Kennel and Daycare property for this care as deemed necessary.

• I understand that my dog will be exposed to other dogs in this group housed facility and therefore may be at risk for illnesses such as canine cough. While Yellow Dog Kennel takes every precaution to prevent this from happening, I understand it is a risk and I accept said risk. I understand that I am responsible for any and all veterinary costs associated with said illness. 

• I understand that I am responsible for my dog's actions while boarding. Should my dog cause bodily harm to another dog or an employee or owner of the kennel, or damage to the facility, I am legally and financially responsible for those actions. 

Boarding Contract

1. Yellow Dog Kennel, its owners or staff do not assume nor shall be held responsible for any liability regarding the animal listed in this agreement, of any kind, character, or nature whatsoever, arising out of, or from, the boarding or care of this animal. This includes any damages which may accrue from any other cause including but not limited to, loss by fire, theft, elopement, injury to persons, animals or property, unavoidable causes, or death or injury to any other animal caused by the herewith named animal during the term of this contract, whether this animal be on the premises of Yellow Dog Kennel and Daycare or not. The owner of said animal agrees hereby to be solely responsible for any and all acts of behavior of said animal at any time within the term and time for the contract. In no case shall Yellow Dog Kennel and Daycare be held, in any way, liable or responsible for the actions of this animal. 

2. The owner(s) of the within named animal, specifically represents that he/she is the sole owner of said animal and that there is not now any lien against said animal. 

3. The owner represents that the within named animal has not been exposed to distemper or rabies within the last thirty days, the required annual license has been obtained and all vaccines are current. The owner represents the animal has not been ill or injured within 10 days prior to their stay. 

4. Yellow Dog Kennel and Daycare shall have, and is hereby granted, a lien on the aforesaid animal for any and all unpaid boarding and/or other charges (i.e. veterinary, food) resulting from the boarding of said animal within the kennel. The owner hereby agrees that, in the event the monthly or weekly boarding charges are not paid within thirty days after they become due and payable in accordance with the terms of this contract, Yellow Dog Kennel and Daycare may exercise its lien rights, and ten days after notice to owner, may dispose of said animal for any and all unpaid charges, at private or public sale, and owner specifically waives and claims if such sale does not secure a price adequate to pay such costs of board and/or other charges delinquent plus costs of sale, then owner shall and must pay the difference to Yellow Dog Kennel and Daycare. Any monies realized at such a sale, over and above the charges due and cost of sale shall be returned to the Owner. Notice shall be conclusively deemed to have been given pursuant to this paragraph if notice in writing of such intended sale shall be mailed by registered mail to the owner of the within named animal at the address given herein, and no further notice shall be required. 

5. If the animal becomes ill or Yellow Dog Kennel and Daycare staff suspects the animal is ill, the owner, or emergency contact, should the owner be unavailable, will be notified immediately. Yellow Dog Kennel and Daycare reserves the right to administer medication, perform medical care and/or seek immediate medical evaluation without owner consent if the animal’s health deems it necessary to act quickly. 

6. Unless owner files a written demand for any claimed injury or damages resulting from the boarding or care of said animal under this contract within thirty days from the date the animal was removed from our care, said owner shall and does hereby waive, any and all rights he/she may have against Yellow Dog Kennel and Daycare for any liability arising under this contract, for damages, or otherwise. 

7. The singular, as herein used, means also the plural, the masculine gender means also feminine gender. If any word, sentence, or section of this agreement be declared invalid, such ruling shall nevertheless not affect any other word, sentence or section. The remedies hereunder are cumulative to Yellow Dog Kennel and Daycare and not alternative. No person is authorized by Yellow Dog Kennel and Daycare to change or waive any of the terms or conditions of this contract and Yellow Dog Kennel and Daycare will not be bound by any changes therein, whether oral or written. All terms and conditions of this agreement shall be binding on the heirs, administrators, of the owner of the within named animal. This contract is the entire agreement between the parties and supersedes any other document and/or oral representations and/or agreements. Time is of the essence hereof. 

8. If action is to be instituted by Yellow Dog Kennel and Daycare to enforce this contract, the owner promises to pay such a sum as the Court may set as attorney's fees. 

9. The owner guarantees payment of this bill. If for any reason this bill is not paid when presented, and is placed in the hands of an attorney for collection, the dog owner agrees that a reasonable fee may be added for attorney’s fees, and other such costs as the court may allow. Interest charged on an overdue bill shall be at the maximum rate allowed by law. I have read this boarding contract in its entirety and agree to the terms herein.
